Additional Learning Support Questionnaire

Following your application to join Brooklands College you provided us with some information about possible support needs and we are now asking you to provide us with more details. Please complete and return this form. So that we can support you fully, please give us as much detailed information as possible. This form may take around 15 minutes to complete and you may want to ask your parents/carers for help in doing it.

Please complete the Additional Learning Support Questionnaire if you:

• Have an EHCP
• Have a Learning Need or Learning Disability
• Have Special Education Needs (SEN)
• Have a medical need that requires additional support for your learning
• Have received exam support in your previous educational establishment (this could be extra exam time, access to a smaller room, using a computer instead of writing, support with reading or writing)
